LATEST FORMS FOR DECLARATION AND REGISTRATION OF FOOD PRODUCTS

1. Form No. 01: Product Standard Declaration

This form is used by organizations and individuals to declare the applied standards for their food products.

  • Key Contents:

    • Information of the declaring entity: Name, Address, Business Registration Number, and Food Safety Eligibility Certificate.

    • Product Details: Name, ingredients, shelf life, packaging specifications, and manufacturing facility.

    • Applied Standards: Reference to Technical Regulations (QCVN), National Standards (TCVN), or In-house Standards (TCCS).

  • Commitment: The entity legally guarantees the product's safety and compliance with the declared standards.

2. Form No. 02: Product Declaration

A more detailed technical form focusing on quality specifications and safety requirements.

  • Key Contents:

    • Detailed quality indicators and safety requirements.

    • Product Labeling: Attachment of the official or draft product label.

    • Technical Explanations: Tolerances for vitamins/minerals and quantitative composition analysis.

  • Usage: Mandatory for products requiring a rigorous quality assessment before entering the market.

3. Form No. 03: Registration Dossier for Health Supplements

This is a comprehensive dossier structure specifically designed for Health Supplements (Dietary Supplements).

  • Dossier Components: Includes the Product Declaration (Form 02), Certificate of Free Sale (CFS), GMP Certificate, and Scientific Evidence supporting health claims.

  • Technical Reports: Requires a Stability Study Report (including long-term and accelerated testing) to determine the product's shelf life.

4. Form No. 04: Registration Dossier for Special Purpose Foods

This dossier applies to Medical Foods, Foods for Special Dietary Uses, and Nutritional Products for children up to 36 months old.

  • Key Requirements: Focuses on scientific data relevant to the specific target consumers (e.g., patients, infants, or people on special diets).

  • Compliance: Requires certification of manufacturing systems such as HACCP, ISO 22000, FSSC 22000, or BRC.

  • Stability: Similar to Form 03, a robust stability study is required to ensure safety during distribution and use.
